It is easy to refer your patient to Functional Therapy and Rehab! Please fax the following items to us at 888-590-4966 and we will take it from there!
- Outpatient Physical Therapy Prescription
- Referring providers’ most recent office visit/encounter note including medications
- Patient demographics and insurance sheet (please see our service area and insurance page as needed)
Once we receive this information we will check insurance information and place an introduction phone call to your patient to verify all their details. Once completed, one of our licensed therapists will reach out to them for scheduling usually within a few days!
Referring providers can expect to receive an initial evaluation/certification to review and sign, shortly after the patients’ first visit. We pride ourselves on ongoing communication during the episode of care through progress reports and if needed, a phone call.
Additionally, unlike other clinics, patients have direct cell phone access to their personal physical therapist during the episode of care for any questions or concerns.
