Scheduling Policy
Functional Therapy and Rehab prides itself on being able to come directly to your home or office to deliver the best and most convenient care possible. Your care is assigned to a therapist that services your GENERAL geographic area. Please keep in mind that we must efficiently travel through the busy metropolitan area to be able to serve all our patients. It is likely that the therapist will be in your area 2-3 times per week but not every day. Please make any of your preferences, fixed commitments, and doctor’s office visits known as soon as possible. This will assist us in offering you the scheduling options that best fit your schedule. Additionally, if you are flexible, please let us know that as well.
Schedules are typically done 1-2 weeks ahead of time. While we prefer to maintain a regular day and time with each patient, there will likely be some modifications based on the needs of the practice as well as new patients entering the schedule. Please allow 10 minutes on the early/late side of your appointment time for the therapist to arrive.
If severe weather conditions are expected, your therapist will call you as soon as possible to discuss re-scheduling options. Thank you in advance for your cooperation and flexibility.
Cancellation Policy
Functional Therapy and Rehab requires at least 24-hour notice to cancel or reschedule a visit with the therapist. Please call your therapist directly or our main number at (617) 870-1082. Your appointment time is specially reserved and worked into the travel plans of the therapist for that day. With short notice, we are not able to re-schedule another patient in your treatment time slot. It is not our intention, but please be advised that you may be subject to a $50 cancellation/no show fee. Thank you for being considerate of your therapist’s time.
Home Health Care
Functional Therapy and Rehab is an OUTPATIENT Physical Therapy practice that makes house calls for your convenience. We CANNOT coincide with ANY Home Health services, including a visiting nurse or therapist. It is part of your responsibility to let our office, or your therapist know immediately if you are currently, or may be receiving Home Health services. Otherwise, you will be liable for any overlapping therapy visits at $150 for evaluations and $120 for follow up visits.
Co-pays/Co-insurance/Deductibles
Co-pays/Co-insurance/Deductible payments are due at the time of your therapy visits. Our office will do its best to inform you of this information prior to your first appointment, but it is your responsibility to understand your insurance benefits. Your therapist will not be knowledgeable about this information. Call the number on the back of your insurance card and ask about your OUTPATIENT Physical therapy benefits, if you need assistance.
It is important to have your payment ready for your therapist to collect at the START of your session. This can be given while your therapist is following up with you since your last session. It allows your therapist to spend all their time on your care. Checks are payable to Functional Therapy And Rehab. Venmo payments may be sent to @sandip-vyas (last 4 digits 2043) and Zelle payments to (571) 275-2043. We do not accept cash or credit card payments at this time.
Financial Responsibility
Our office will bill your insurance company for services rendered. This is based on the insurance information that you have given us and/or information that you have verified (if it was received from your doctor’s office). Any charges that are not covered by your insurance company will be your responsibility. Please be aware of your insurance benefits.
Insurance Referrals
Most of the time, especially as a courtesy to our seniors, our staff will make an initial attempt to secure an insurance referral/authorization if needed for therapy services. The insurance referral/authorization is placed by your primary care doctor. If we are unsuccessful, we may ask you to assist in the process. It is ultimately your responsibility to check with us, your insurance, or your primary care doctor, to ensure a referral for PT is in place based on your health plan’s requirements. Otherwise your treatment will be subject to the current private pay rates of the practice.
